The successful applicant will receive support, training opportunities and clinical supervision to be able to develop their career whilst working in a fast paced and challenging environment.

Practice Nurse Requirements:

• Knowledge or experience of Chronic Disease Management
• The care of patients presenting with undifferentiated health problems
• Knowledge or experience of Chronic Disease Management
• The care of patients presenting with undifferentiated health problems
• Child and adult immunisations
• Cervical Cytology (Not essential)
• Asthma and COPD monitoring (Desirable)
• Family planning
• Ability to work as part of a multidisciplinary team
• Up to date knowledge of QOF and Enhanced Services
• Be fully up to date in terms of clinical updates, immunisations and vaccinations

Description:

We are looking for a Nurse Practitioner in Southchurch to join our team who will provide holistic and also quality care to our patients.

The duties of a Nurse Practitioner include assisting with all areas of patient care, including diagnosis, treatments, and also consultations. Second, they understand that there is no single solution to patient care. Please include your previous related experience with your application if you appreciate working in a collaborative setting and with all parts of patient care.

Responsibilities

• Perform initial patient screenings and patient history and physical examinations
• Update patient records and check records for accuracy at each patient appointment
• Analyze test data and determine the need for follow-up appointments and further treatment options
• Deliver quality care while maintaining all company metrics pertaining to productivity
• Serve as the resource patients need for ongoing care information, counselling and also provide guidance for any patients with critical conditions

 Qualifications

Masters in Nursing

  Skills

•   Must hold current license/registration to practice

•    Excellent verbal and written communication skills
•    Must have the ability to exercise sound judgment and discretion
• Ability to maintain highly confidential information

Experience

• One must Registered Nurse(RN)
• NP-focused graduate master’s or Doctoral Nursing Program
• Successfully Pass a National NP Board Certification Exam
